Follow an evolving trainee case and apply the three-tier escalation pathway as the evidence changes.

ESCALATION BRIEF

Three levels. Different thresholds. Different responsibilities.

Use the evidence available at each point to decide the proportionate response. The case does not require every tier simply because it appears in the SOP.

Follow Dr. Youssef’s case as it evolves. At each point, decide what the trainer should do now based on the pattern, seriousness, patient-safety implications, professionalism, and due process.

Tier 1

Structured coaching, documentation, and continued observation.

Tier 2

Coordinated remediation when a concern persists despite Tier 1 support.

Tier 3 / Red Flag

Programmatic escalation when the nature or seriousness of new evidence crosses the escalation threshold.

Work through the case in sequence. A Red Flag is one possible response within the same decision set; judge it by the evidence, not by its label.
